Ready for use: Request form for COVID-19 related leave taken on or after January 1, 2021

The Families First Coronavirus Response Act (FFCRA), that provided paid leave to employees for COVID related reasons, expired on December 31, 2020 and is no longer available.

Instead, employees can use accrued leave time to cover absences, or they may request leave without pay (See section 4 of the Request for COVID-19 Related Leave form for more info).

All leave programs require the form (and any documentation) be emailed to the CALS Divisional Disability Representative (DDR) at

Requests will then be reviewed and either approved or denied. This notification will come via email and provide additional information on next steps.  Please provide as much information as possible and any documentation.
