Vaccination information for CALS employees
How campus is conducting vaccinations
Starting March 15, employees need to use their MyUHS account (go.wisc.edu/myuhscovidvax) to access eligibility information and appointment options. Employees enter their NetID, password, and date of birth, and a series of prompts will guide employees to determine eligibility to receive the vaccine on campus. Please know campus has a limited supply of vaccines, which is why they have the prompts to confirm whether employees are eligible for the vaccine through UHS. Eligible members of the campus community may also seek out a vaccine appointment through their health care provider or other off-campus providers.
I have been vaccinated off campus and want to upload my vaccination record to UHS
If employees were vaccinated off campus, they should submit proof of vaccination by following the instructions at this website: https://covidresponse.wisc.edu/faq/i-was-vaccinated-off-campus-how-do-i-let-uhs-know/. Instructions are:

- Find your vaccination record in the Wisconsin Immunization Registry, or find the vaccine registry in the state you received your shots.
- Select “print” and save your record as a PDF.
- Log in to MyUHS.
- On the bottom left side of the page, click on Immunization Upload.
- Upload your immunization record
Please know, it may take up to five business days for the record to be reflected in an employee’s MyUHS account.
