Time for annual Agricultural Research Station respirator fit tests

The 2020 Agricultural Research Station (ARS) respirator fit tests at the Arlington and Hancock research stations have been scheduled. See below for specific details. Anyone who is required to use a respirator needs to complete a fit test. The fit test is essential in ensuring the respirator provides an acceptable fit to the wearer. As a reminder, anyone who is scheduled to complete a fit test should complete the ARS pre-outreach instructions steps copied below before the scheduled fit test dates.

Step 1 – Release & Consent Forms – (only for those that haven’t been fit tested before)

  • Login to MyUHS: “myuhs.uhs.wisc.edu”
  • Click on “Forms”
  • Complete OCCUPATIONAL MED: 1. & OCCUPATIONAL MED: 2.

Step 2 – Respirator Medical Evaluation Questionnaire

  • NOTE: Not everyone needs to complete this questionnaire —please check email from EOH to see your status
  • NOTE: Be sure to complete the questionnaire within 20 minutes – the system will time-out after 20 minutes
  • Login to MyUHS: “myuhs.uhs.wisc.edu”
  • Click on “Messages”, Click “New Message”
  • Select role, select “I am a UW employee…”
  • Select “RESPIRATOR MEDICAL…”
  • Complete & submit

Step 3 – Fit Test Appointment

  • Please email madeline.wojciuk@wisc.edu to confirm your attendance
  • Submit forms & questionnaire >5 days before apt
  • Do not eat or smoke 15 minutes before appointment
  • Be clean shaven (shave your face the morning of your appointment); EOH cannot fit test anyone that has facial hair that touches the respirator seal
