Now that your biometric screening is done, have you completed the final step to receive your $150 from Well Wisconsin?
After your biometric screening you still need to complete your health insurer’s health survey, and instructions on how to do that are outlined in step 1 below. If you have not yet registered for a biometric screening it is not too late, follow the instructions outlined in step 2.
- Complete your health insurer’s health survey by clicking on your insurer’s link at http://wellwisconsin.wi.gov.
- Complete a wellness assessment onsite or with your physician. To get the most accurate results, it is recommended to fast for 8 – 12 hours prior to your appointment. Option 1: Register for a free appointment (required) at https://register.wellness-inc.com/stateofwi. The appointment will take less than 30 minutes. Please bring your health insurance member ID with you to your appointment. You will receive your current cholesterol level (total, HDL, LDL, triglycerides), blood pressure, blood glucose, height, weight, Body Mass Index (BMI), and waist measurement. Option 2: If you want to complete the assessment with your physician, click on your insurer’s link at http://wellwisconsin.wi.gov to learn the correct process. Please note, there may be fees associated with having labs drawn with your physician.
