NIH Change to Fellowship application signature requirements
The National Institutes of Health (NIH) and other Public Health Service agencies have revised their signature requirements on Form 416-1 and 416-9. In preparation for the transition of these forms to Grants.gov, the PHS has removed the signature requirements for the fellow & sponsor from these forms. In return, the University is required to maintain on file an assurance from both individuals ensuring compliance with certain PHS policies. The new requirements take effect December 2008 (see NIH Announcement NOT-OD-09-007 http://grants.nih.gov/grants/guide/notice-files/NOT-OD-09-007.html).
In order to comply, the University has created a form that must be signed by both the fellow and the Sponsor prior to submission of the application. The form is available online at: http://www.rsp.wisc.edu/forms/HHS_Fellowship_Assurance.pdf. This form must be signed by each individual and uploaded to the WISPER record for permanent storage prior to submission of the application.
